Regarding your concern, I would like to explain that her weight loss seems to be related to her polypoid esophageal mass and mass on chest (reviled by the performed fibrogastroscopy and chest x ray).
It is very important to perform as soon as possible a chest CTscan, some tumoral antigen levels in her blood (like CEA, CA19.9) and a biopsy of the polypoid mass or the mass on the chest, if possible to determine the diagnosis. It may be a primary tumor of the chest or of the esofageal and a metastasis or two incidental findings (which would be a rare coincidence). I recommend consulting with an oncologist or a surgeon on the above tests to establish the diagnosis and find a possible therapy.
I would like to explain that time is a very important issue in her delicate situation, and in case of a malign tumor, a chest CT scan would be urgent (and not wait until the date of booking).
The weight loss may be caused by the difficulty in swallowing by the mechanical obstruction of the esophagus or the toxins in her blood (in case of a malign tumor).
Regarding the findings on her brain CT scan, I would say that they are small strokes, caused by a chronic small vessel disease of the brain, which is a common finding in a 74 year old diabetic and hypertensive patient. The weight loss is not related to these findings.
Her low blood pressure, may be related to dehydration or to her general situation (always related to the esophageal and chest mass).
Although I recommend to closely monitor her blood pressure and if her blood pressure is always low, it may be necessary to stop taking Ridaq.
