Question: I did the three day Dofedilide "loading" at University Hospitals last week while in a-fib. It converted me to sinus, and I was released on the 5th day. On the 8th day I returned to afib. I throw up each day (once) have a horrendous headache and don't sleep well at all. I'm going to disscontiue use of it. Besides "you should consult your doctor," what are your thoughts? Thanks you for your time, Doctor

Brief Answer:
There are other antiarrhythmic drugs to be tried

Detailed Answer:
Hello and welcome to HCM. I understand your concern. Thank you for your question.

I can see from your medical history that some of the antiarrhythmic drugs have been tried on you. These bouts of vomiting and nausea are indeed a side effect of dofetilide. Therefore, my opinion is also that it should be discontinued. Let me offer you my insight.

A class III antiarrhythmic drug, amiodarone (Cordarone), is a very effective drug in controling this atrial arrhythmia and in preventing further bouts from happening. If you were my patient, I would consider to put you on that drug, plus associate it with a beta-blocker, which is helpful in rate control and, also, in preventing from further episodes. Also, I would recommend you to undergo an echo cardiogram, to assess the structural integrity of the heart valves, also the size of the atrial chambers, in order to further decide about the strategy of treatment and follow-up.

I hope I was helpful. I'll be happy to help, if you have follow-up questions.

Best regards,
Dr. Meriton
