Question: I Have a hard pimple like knot about 4 inches below my belly button ..it is swollen..very hard feeling ..not soft inside ..an appears to have a head ..very painful as its right at my waist line

This symptoms are caused by some kind of infection. It may be due to sebaceus cyst or ingrown hair. Pain and redness are quite characteristic for an infection.
The white spot you are able to see can indicate some pus accumulation.
I suggest you to consult general surgeon as soon as possible. Soft tissues ultrasound should be done. If any pus collection will be found, should be incised and drained. Antibiotic seems also necessary.
Please do not wait with it.
