Suggest Treatment For Osmotic Demyelination Syndrome And Hyponatremia

Question: Hi Doctor,

My father was diagnosed with Extra-Pontine Myelynolysis / osmotic demyelination after CABG and an episode of hyponatremia. He has also been diabetic for ~15 years.

The doctor here says it will just require long term care and not recommending any medicines, but I suspect my father would never be cured or may get worse.

He currently is bedridden, has strength in hands and sometimes is able to sit up on his own. He is not able to speak, sometimes speaks with a lot of difficulty. He recognizes us and can answer in yes/no when asked specific questions.

We are currently in a known hospital in XXXXXXX Can you please advise, what options do I have?

Here,I would just like to agree with the treating doctor because 'Extra-Pontine Myelinolysis / osmotic demyelination' is such a clinical condition where no medicines can make the things better overnight.It is the natural healing process of the brain tissue which will improve the situation over the time.Supportive care is the mainstay.Physical rehabilitation plays the key role.

And of course,the condition may take from months to a year to improve.No specific medicines as such for this condition.

I have read about the Supportive care and Rehabilitation terminology at so many places. What exactly is needed in terms of Supportive care and Rehabilitation?

Should we look for specific treatment in a Rehabilitation center or care at home with advise from Physiotherapist?

Can you please elaborate?

Rehabilitation refer to all the strategies which help to make a patient physically,mentally,socially and occupationally independent(or less dependent for that matter).
Physiotherapy can help to achieve the goal of physical rehabilitation.If speech is impaired,speech therapist's role comes into play.
A Psychologist or a psychiatrist's opinion may help to achieve morbidity associated depression,and help to achieve mental rehabilitation.
Achievement of these goals help to the goal of social and vocational rehabilitation.
