Suggest Treatment For Heat Boils Around Anus

Question: Hello Doctor, I am 35 years old, having heat boil at anus, it got busted almost 15days back, but often releasing puss, like sometimes it looks like nothing is there and sometimes pain and puss, when I feel pain it looks like another boil formed, 3 days back there was so much of puss released, now I am using antibiotics(prescribed by a general surgeon), but still it is releasing some puss often. Is there anything I need to do more? or anything I am missing out? Please suggest.

To recapitulate: 35 years - boil at anus - busted 15 days back - off and on release of pus - sometimes nothing is there - now taking antibiotic as prescribed by the General Surgeon - Wants to know what more to do - anything missing ..

This is the most tricky area as for the diagnosis.

Now in this situation the possibilities are:
- Ischio-rectal fossa abscess drained inadequately.
- Fistula-in-ano

I would suggest the following:
- We used to go for fistulogram in older times - now we go for MRI study. This helps to know the possible diagnosis beforehand.
- Exploration of the abscess surgically to see and diagnosed, do probing, if lower type of fistula go for a definite surgery.
If Ischio-rectal abscess - Cruciate incision to allow the healing from within, not allowing the skin to heal before internal healing occurs.
- If high anal fistula, well multiple therapies, theories.

I have tried to explain as per the books, experience and what possibly may be in your case as per the history your have given.
Please discuss all these points with your General Surgeon, you shall be well diagnosed and treated well towards ''cure''.
