Detailed Answer:
Hello

Welcome to HCM

The rashes in diaper area can be due to contact dermatitis, candidial (fungal) dermatitis or secondary bacterial infection in contact dermatitis.
As per your description it appears that there was contact diaper dermatitis that responded to diaper rash cream but the lesion that is persisting can be due to the secondary infection.
As diaper rash cream contains zinc oxide but not any antibiotic this lesion due to secondary infection is not healing. Here application of antibiotic ointment would help.
The skin lesions are better diagnosed by direct examination. So I would request you to attach an image so that I better able to help you out.
