Based on the facts and report of your ultrasound scanning that you have posted your pain in in penis and problem in urination seem to be due to enlarged Prostate .
I shall be happy if you can post me more details to help me in making precise comments
1) How is your urine flow whether poor ,moderate or normal ?
2) Do you have other urinary symptoms like frequency for urination ,nocturnal frequency,hesitancy or urgency etc?
3) Do you have pain in perineal region ?
4) Do you have pain while ejaculating ?
5) Are you Diabetic?
Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
Further treatment will depend upon result of these tests and final diagnosis.
In the mean time continue taking Urimax D once daily along with Nitrofurantoin twice daily .
