In view of acne and open pores, I would recommend a salicylic acid based face wash for regular use on daily basis avoid using moisturizer as it could aggrevate breakouts. Drink around 3 to 4 litres per day. In case of itchy scalp and dandruff present which could also cause acne breakouts use Ketaconazole shampoo weekly thrice for 3 weeks. I would recommend vitamin C supplements such as limcee tablet one per day for 3 months which helps with the skin healing.
