Question: My wife has a swelling on the inside of her lower lip. It looks like the start of a large canker sore or blister which has fluid in a sac that has not opened. It has been there about a month. Must she see an oral surgeon or would an M.D. be the right one to consult. Her dentist suggested that a biopsy might be in order.

it can be a mucocele that occurs secondary to any trauma like accidental bitting over lips.
The other possibility can be a irritational fibroma, that occurs secondary to chronic irritation to that area by the teeth.
Nothing to be panic, consult an oral physician and get it evaluated. Once the diagnosis is confirmed you can get it surgically removed.
These are the possible causes, diagnosis and treatment plan based on the description that was provided by you.
Regarding your insurance, if it is not covered if you get it treated by the oral surgeon, you can get it done by a general surgeon. But oral surgeons are ideal to treat these lesions.
