Brief Answer:
Surgery is done when medicines don't help

Detailed Answer:
Hello,
Thanks for posting your query.
There are many causes for pain with swelling in a single knee joint at this age group. Most common cause is Osteo-arthritis of knee joint.
For osteoarthritis, weight reduction is the main treatment along with medicines to decrease pain like NSAID's - paracetamol, aceclofenac etc., Life style modifications like avoiding sitting cross legged, squatting, using XXXXXXX type toilets will decrease the rate of progression of osteoarthritis. Exercises like quadriceps strengthening, swimming, straight leg raising will help in improving the muscle strength around the knee and will help in decreasing pain.
In case the medicines do not help then surgery - knee replacement is considered . It is a safe surgery with good recovery . The other knee also may get affected after a few years as it is an age related degenerative change of joints .

I hope this answers your query.
