Hello and welcome to ask a doctor service. I have gone through your query and viewed the image.

It seems like an inflamed follicle which is usually a bacterial infection of follicles. It is unlikely to be Herpes labialis though I suggest you to take a blood test for HSV ELISA type 1 to rule out this possibility. In the mean time I suggest you to use a topical antibiotic e.g either Neosporin ointment or Mupirocin ointment, twice daily.

Herpes labialis usually presents as a grape like cluster of fluid filled but it may initially start just as a red, raised area which later becomes a cluster of fluid filled lesions. You may anyways use a topical antibiotic ointment over it. Is this happened before?
