Question: I have constant low back pain that extends into the groin. I'm concerned that it may be more than arthritis. Should I be concerned. Could it be a neoplasm?

Hi,

It is understandable to be concerned re persistent pain in any area of the body.

In terms of the back, the most common reason is arthritis but there are other possible causes of back pain.
Cancer of the back is one cause but usually cancer in the back is due to spread from another area, it is usually metastatic.

If you have a history of primary cancer elsewhere then unfortunately , spread to the back is possible.

As mentioned however, arthritis and similar changes to the spine, are the most common causes.

To find the exact cause, you would need to be seen by your doctor. In addition to physical examination, you would need imaging of the back, an MRI would be best because it would show both the bones and other soft tissue.
An X-ray would only show bony changes , it would show arthritis and would possibly show a mass if one is present.

It is best to be seen to find the cause of the pain
