Is Intrasubstance Laminar Tear Of Subscapularis In An MRI On My Right Arm A Serious Concern?

default
Posted on Thu, 29 Feb 2024
Question: I just had an MRI on my right arm. I don't see my orthopedic doctor for another month. Wondering if you can help explain the MRI results in layman's terms before I see my doc. Test says "Intrasubstance laminar tear of subscapularis measuring 0.8 cm in SI and 1.5 cm in ML dimensions. Perching of biceps long head tendon into subscapularis tendon." What does that mean? Also, fyi, I had the MRI because I have biceps pain if I lift anything over 5 or 6 pounds. (normally I can lift 20 pounds biceps - curls - just fine) I can still do triceps weights without pain.

Brief Answer:
Tear of muscle and displaced tendon.

Detailed Answer:
Hello,
Thanks for posting your query.
You have a small laminar tear of subscapularis muscle. The perching of long head of biceps tendon suggests its displacement which is painful and does not allow you to lift the weights. The tear is within the substance of the muscle and can heal with rest and immobility. The perching should come to normal after the muscle heals. This can heal with proper physiotherapy exercises after the acute pain subsides.
I hope this answers your query.

Thank you Dr. Tayal. Two follow up questions, just trying to understand terms. What does "laminar" mean? And, what does "perching" mean in this context?

Thank you - XXXX

Brief Answer:
Details below.

Detailed Answer:
Hello.
Thanks for writing again.
Laminar means few fibres of subscapularis are torn. It is not a full thickness tear. Perching refers to the dislocation of the tendon due to torn subscapularis muscle. The tendon head is displaced and perched on the torn muscle.
Hope my answer is helpful.
