Question: A 64 year old male with Diabetes and Hypertension from 7 yrs on medication .HB A 1c 6.3 , FBS 124 Urine Sugar +, and ECG changes of LVF and T inversion . Report uploaded .
What could be the immediate and long term prognosis and complication , what additional investigation will be required to assess .

Detailed Answer:
Hello ,
I am Dr. Mody and I would be addressing your concern .
With HbA1c at 6.3 the blood sugars are well under control and Diabetes continue the same medication .
I have reviewed the ECG ,
These changes can be due to long term hypertension causing some thickening of the heart muscle .

In either case , considering his age and the risk factor like hypertension and diabetes , he falls in intermediate to high risk for future coronary heart disease .
No gross short term issues if both diabetes and hypertension is under Good control .

The tests which will help to further prognosticate would be 2 D echocardiography of the heart , if normal Treadmill stress test ( Stress echocardiography preferred in his case ) and in blood investigation a Lipid profile , S. creatinine ,
Urine for microalbumin should help us diagnose his exact status
