Question: I have right temple pulsating and right eye pain and pressure which goes into my ear and I feel nauseous and dizzy light headed and at times blurred vision right eye waters when the episodes happen. Been happening on and off for several years but has got to the point of unbearable

There are different possible causes for the pain that you are experiencing. Some of the more common possible causes would be: (1) a type of migraine- there are different types of migraine, your symptoms can occur in the classical type of migraine

typically migraines are associated with pain occurring on one side of the head, it is pulsating, associated with nausea and can be associated with visual changes as well. Diagnosis requires at least 5 episodes,lasting at least 4 hours.

(2)Cluster headaches are headaches that occur on one side of the head, the pain occurs in clusters with a person having 1 to 2 clusters per year with each cluster (time having pain)- lasting anything from 2 weeks to 3 months.
Watering of the eye or redness of the eye or even nasal symptoms can occur.

(3)sinusitis- usually there are additional symptoms such as sneezing, nasal symptoms , post nasal drip

(4)very rarely it may mean a mass of lesion in the brain- this is not as likely since you state that the symptoms have been ongoing for years

(5)neuropathic pain. Pain can also originate from abnormal function of the nerves in the area, this is called neuropathy and the pain is neuropathic pain. There are different causes of neuropathic pain eg liver or kidney disease, diabetes, trauma to the nerves, vitamin deficiency (b12, folate), MS, infections etc

It would be best to see a neurologist for assessment. Imaging such as CT would be needed to rule out a structural cause.
Treatment would depend on the exact cause but there medications that can help the conditions listed either normal pain medications or migraine specific meds (which are also used in cluster headaches) or sinus meds
