Question: I have episodes where I am so hot I start sweating, I feel like I am in an oven. Then I have episodes where I am very cold almost shivering. My legs are very weak and a tiny amount of exertion leaves me breathless.I feel bad almost all of the time. I have no energy.

I carefully read your query and here is my advice:

It is very important when a patient has these episodes of heat and cold to make sure it is not due to fever. I would advice you to check your temperature while you feel hot as in general if fever develops it is highest when you start to feel hot. If your temperature is over 38.0 C (100.04 F) then you do have fever and it is important to find the cause of it.

Other possible causes of the episodes of feeling hot and/or cold might be due to hormonal changes. Menopause is a common reason and also thyroid problems. I would advice you to have a TSH and FT4 test done to make sure of the thyroid function.

To conclude:
- Check your temperature to rule out fever as a possible cause of the episodes. If temperature is high for longer than 3-4 days, you will need to see your doctor and have a thorough examination to determine it's cause
- If the episodes have been there for more than a few weeks, and you do not have fever, you can consider having TSH and FT4 blood tests to rule out problems with the thyroid gland
- Sometimes few other tests might be needed like heart and lung examination, a CBC and ERS as a general check especially because you are tired and feel breathless with small excertion.
