Question: I had two irregular menstrual cycles. One in April and one in November (spotting for a week, then nothing, then maybe a light period 21 days after the last--Nov. 2nd). I have attached the TVUS report from April. How frequently do you recommend one have a repeat TVUS?

Is a DNC needed? My understanding is that women can have longer cycles when they are 50. I don't understand why pills or DNC would be necessary if the bleeding isn't heavy. Can you comment on this?

I read your query and here is my advice.

I see the ultrasound report you uploaded and the uterus is enlarged and endometrial lining is thickened.
The spotting is repeated in November and you are right when saying that it is common in women around 50 due to hormone imbalance of perimenopause.
The D&C procedure is done in order to check if there are any pathologies causing the spotting rather than a simple imbalance.

A thickened endometrial lining and spotting may be caused even by endometrial abnormalities or polyps and D&C is needed to provide biopsy material in order to be sure that these pathologies aren't causing the spotting.

Hormone replacement is needed to stop spotting which may make you uncomfortable.

The doctor said my ultrasound from April was fine. She didn’t say the lining was thickened at 11.

Wouldn’t a biopsy be more appropriate than D&C?

D&C is a procedure when the column uteri are enlarged, opened and material is provided for endometrial lining for biopsy.
