Question: I had an MI in 2002 December. Subsequently I have good LVF - heart pumping is good - EF 60 percent I and have been on regular medication. A few PTCAs with drug eluting stents followed, the last being one year ago. I have to go for a surgery to treat my deviated nasal septum next week . Please suggest some precautions , dos and don'ts; and please throw light on any risks that are involved

I have gone through your query and here is my advice.

As per your history, I would suggest you...

1. You have to stop tab. Clopitab A i. e. Blood thinner at least 5 to 7 days before surgery after cardiologist consultation.

2. Kindly consult with your physician for detailed clinical examination. He may procure Electrocardiogram/2D Echo, blood pressure monitoring, blood sugar levels, complete hemogram, thyroid profile for surgery fitness.

3. You should not take anything by mouth from midnight one day before surgery. ( provided surgery to be planned in the morning)

4. On the day of surgery, You should not take insulin injection and Tab. Tendia by your own. Physician will adjust insulin dosage as per fasting blood sugar levels.

5. After surgery, kindly avoid forceful nose blowing/coughing or heavy weight lifting/heavy exercises for few days as per ENT surgeon opinion. Along with that watch for symptoms heavy nasal bleeding, severe pain, fever.

Every surgery carries some risk. So I would suggest you consult with cardiologist and physician and undergo pre-surgery fitness assessment. Only after complete assessment including anesthetist, you may go ahead.
