Question: I had a hydrocelectomy years ago on left testicle. Last week there blood in semen. Two days later after next orgasm left testicle became painful, been in pain since then. What's cause and what can be done

Noted the history well and understood the concerns.
Hydrocelectomy is in no way connected to the present problem.
Blood in semen along with pain in the left testicle indicates that there may be an inflammation of epididymis and.or testicle.
This can be secondary to urinary tract infection with or without prostatitis.
With the age of 63, we need detailed evaluation as follows:
Tests of blood for CBC, sugar, kidney function and as advised, PSA that is prostate specific antigen.
Tests of Urine: routine, microscopy, culture and sensitivity.
Ultrasonography of abdomen for genitourinary system, of scrotum with color doppler mode also.
Per-rectal digital examination, clinical examination of scrotum.
Tests of Semen.
Consult your Doctor, get investigations done, get a reference for an Urologist for proper diagnosis and management.
All these things asap.
