Question: I am a breast cancer survivor, aged 35 now.Had cancer last year T2N0M0, grade 1.My cancer is hormone receptor positive and i am on tamoxifen 20 mg per day.I am also BRCA 2 positive, however I have no family history of breast cancer.Although my father had colon cancer and my grandfather had throat cancer.I had modified radical mastectomy of my left breast.I wanted to know that is it necessary to get my other breast removed or i can chose surveillance over it.I am not willing to have mastectomy again.I want to get my left breast reconstructed.Kindly suggest

prophylactic mastectomy not routinely required in opp. normal breast.

Hello and welcome to Ask A Doctor services.
I have read your query and here is my advise.

* As per my surgical experience, routine surgery of opposite breast removal
is not mandatory in a patient with BRCA 2 positive cancer in another
breast who is on Tamoxifen.

* Regular surveillance with oncologist follow up, mammography and self
breast examination is best recommended for normal opposite breast.

* So far as reconstruction of operated part is concerned, best modalities
are available in various directions as autologus graft, skin grafts, flap
procedures and prosthetic implants depending upon the individual case
need.
It is best recommended to discuss with plastic surgeon in details regarding
the reconstructive part as physical examination is a must for that.

I heartfully thank you for your genuine advice.Actually i am in great fear after testing brca2 positive.I read that chances of reccurence are high in opposite breast also, so i was fearful.My oncologist here also suggested same thing as suggested by you.I am continously worried after being brca positive.Fear of reccurence has overpowered by decision making ability.But after your advice i feel better and i will chose surveillance instead of prophylactic sugery
