Brief Answer:
Intestinal obstruction should be ruled out.

Detailed Answer:

Hi,

There are two things: Fatty umbilical hernia and uncomfortable gas that is giving bad gurgling. I can think of the following possibilities:
Gurgling is usually due to excessive gas formation but is then temporary phenomenon. If this is of a long standing then the possibility of intestinal obstruction should be borne in mind. This can be only partial causing gurgling and can be due to involvement of the part if the circumference in the hernia sac getting repeatedly constricted causing gurgling.

Hence, it is important to get the following:
Clinical evaluation by a General Surgeon who on an actual auscultation can know about the intestinal obstruction by typical tinkling and hyperperistalsis. Barium follow through or a contrast enhanced CT scan of abdomen can find such a cause. And if present needs surgery.

Please get these done to get an appropriate diagnosis and have a proper plan of management for cure.

I hope this answer helps you. Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
