Question: Hi, my father is 65 years old. He is borderline diabetic. Around 20 days ago, he had 3 episodes of blood red stools in one day, He took some medicines from the local chemist and stool became normal.
The day before yesterday, he again had blackish lose stools, his blood pressure dipped to 60/40 and he fainted.
He got an occur faecal blood test done. It was negative. He tested positive for round worms,
What could the reason for the drop in blood pressure and black stools?
I would appreciate a detailed analysis.

Immediate medical attention needed
Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.
Bloody stools in a 65 year old could be due to colitis, malignancy, diverticulosis, hemorrhoids etc. Since it became normal with medications, colitis must have been the probable cause.
Black stools especially if sticky and offensive indicates bleeding in the upper gastrointestinal tract. The drop in blood pressure indicates significant bleed.
I would advise you to get him medical attention at the earliest.

He took medicines for parasitic infection day before yesterday. 15 days before he fainted, he got a cbc done, it was normal. He had also got abdominal ultrasound done which was also normal.
It has been 2 days since he passed out, he has taken medicine for worms day before yesterday.
However his stool is still blackish.
What do you recommend as the next step? Stool is blackish but contains no occult blood. round worms are also seen in pathology.

Detailed Answer:
Hi,
Since the blood pressure dropped to 60/40 only day before yesterday and that he is having black stools, I would advise immediate medical attention if not seen already after the above mentioned thing happened.
Though the occult blood test is negative, there was a significant drop in blood pressure and he fainted which needs evaluation.
He needs to be examined again by his physician.
