Question: Hi, doctor. It's been a while since I e-mailed you. You have been a tremendous help. Since u seem very familiar with atypical odontalgia (the dull ache between two teeth where the nerve just continue to fire and aches constantly), my question is this: I've had this for several years now with the intensity going from 3 up to 10. Lately, it's been at a 9 or 10 with that aching traveling into the neck, shoulder, even down the arm into the elbow. Feels like a pulling type ache. Can that damaged nerve cause this? And might that injection you recommended (botulinum neurotoxin type A) mitigate some of this aching? It's just so uncomfortable. Thank you so much.

I have read your question.

That type of pain may be felt locally in the neck, but the shoulder and arm pain are very unlikely to be linked with it.

Shoulder and arm pain may be caused by a pinched nerve in the neck for example, and certain head and neck positions may exacerbate it.

I still think that local injection of botulinum toxin A is an appropriate treatment for that type of pain.

Shoulder and arm pain should be eased also if these are caused by muscle spasms of the neck.
