Hi, I Have Tinnitus, Blockage Feeling And Occasionally Congestion In

Question: Hi,
I have tinnitus, blockage feeling and occasionally congestion in my left ear for a while now.
MRI, hearing test, ECG, blood tests and electro cochlear graphic result were all normal.
My major concernI I get nausea followed by vomiting, dizziness and light headed when ever there is congestion feeling in my left
Ear.
Tinnitus and blockage feeling are present all the time 24/7
I don’t get any of the following symptoms:
Spinning, headache, involuntary eye movements, photophobia or eye floaters.
Diagnosed from different ENT Specialist as ? BBPV, ? Vestibular migraine , menieres
Treatment: Betaserc 16 mg 3 times for the last 3 months with no positive effect.
Fluticasone furoate nasal spray once a day.
Tablet loratedine 10 mg when ever required
Tablet stemetil 5 mg when ever required.
I stopped using the last 2 medications without doctor’s permission, whenever I take them I become pale, very weak and fatigue. I am known case of G6PD.
I am desperately for help and will appreciate your opinion, advise and line of treatment.

Mostly early Meniere's Disease. Other causes less likely but may co-exist.

Hi,

Thank you for your query.

1. From your description, the most likely cause is endolymphatic hydrops, one of the causes being early Meniere's Disease.

2. A salt-restricted diet will help. Unfortunately, acetazolamide (Diamox) should be avoided in G6PD deficiency.
A high dose of betahistine may help.

3. Usually the PTA would show a mild low-frequency hearing loss in the affected ear initially, the dizziness will not be positional and the classic migraine symptoms are not seen as abovementioned.

4. AIED (Auto Immune Ear Disease) should be ruled out if possible.

5. Further testing that can be done are VEMP (Vestibular Evoked Myogenic Potentials), VNG (Videonystagmography), OAE (Otoacoustic Emissions), Impedance Audiometry, Glycerol Test (which can be quite an ordeal but may temporarily reduce symptoms in Meniere's), and Caloric Testing. These can help to further narrow down the diagnosis.

Thank you dr for your detailed answer. I have few more questions:

1.Can food allergy test help us with the diagnosis? Will it help me identify what triggers the symptoms? If so, what type of food allergy test do you recommend?

2. Im assuming you are in XXXXXXX Do you recommend any good doctors or hospitals for diagnosis and treatment.

3. Can it be something related to neck posture or jaw alignment even though i dont have any pain?

1. An ImmunoCap food allergy/IgE test and a Food Intolerance/IgG test may help in some cases.

2. Currently, in any country, there will be very few centers treating tinnitus. Based on the above test results, we can discuss further options.

3. If you clench your jaws tightly and there is a change in the tinnitus, the jaw may be contributing to the tinnitus. The neck will usually be associated with vascular /pulsatile tinnitus, and a change of pillow/s can be tried, especially if the tinnitus is worse in the mornings.

1. Otherwise, could it be vestibular migraine? How can i rule this out?

2. Any homeopathy medicine you recommend for minniers or vestibular migraine?

3. Should i avoid the following: eggs, milk and cereal, nuts?
