Brief Answer:
Regarding scrotal lesions.

Detailed Answer:
Hello and welcome to ask a doctor service. I have gone through your query and viewed the image.

This seems like herpes. It could also be a simple bacterial skin infection. I can see a few erosions in the center and some small satellite erosions nearby. I suggest you to do a screening test for herpes ELISA for HSV type 1 and 2 to confirm or rule out the possibility of herpes. In the mean time I suggest you to use a topical antibiotic e.g either Neosporin ointment or Mupirocin ointment, twice daily for a few days.

Hope I have answered your query. Let me know if you need any more assistance.
