Question: Hi Dr,
I have a hx of svt. Dr was unable to see it during holter a few yrs ago and I now have a loop recorder. I had been symptom free from svt for 3 yrs without medication just using valsalva. I must tell you I am going through early menopause at 38 and assuming my hormones are all over the place. I have recently been getting what feels like a thump in my heart almost as if it is skipping a beat or hiccuping, but this happens at rest and at night mostly. I had ordered a kardia and tried to.get a reading. I have recorded what looks like a pvc. I will upload. This is really affecting me daily and giving me anxiety. Is there something tbat can be done. I dont smoke or drink caffeine. Thank you

Detailed Answer:
Hello

After going through the medical details provided by you I can understand your concern for your health and I would like to tell you that as you are in a young age group and as per your statement these ectopics are affecting quality of life.

If you have your loop recorder report kindly upload that a so would like to review it.

Now coming to management options which you have, first is medical ie with the help of medications that will keep you symptom free but is a temporary solution. Second is, EP/RFA (electrophysiological study and Radio frequency ablation) which is an invasive procedure and has a high success rate of permanent cure.

I suggest you to kindly personally consult your cardiologist and discuss the possibility.

Kind Regards

Thank you for prompt response Dr. I do not have access to my loop recorder reports as they are sent directly to my cardiologist. I have been trying to contact him, but does not have any appts as of yet. I bought the Kardia on my own to see if I can figure out what was going on. I was not placed on meds at the time of svt due to low blood pressure and cardizem made me feel terrible. I was spoken to about the ablation, however nothing was showing up on recorder at that time, it was being missed until I recieved loop recorder and hasnt happened in 3 yrs I have had it. Now I am having what I just showed you.
What is the report of what I showed you, is it a pvc dr? I am no longer having svt for last 3 yrs. Would ablation help with pvc?
Are these constant pvcs dangerous?
Thank you Dr

Hello again

Detailed Answer:
We need to repeat a loop recorder again or a 48 hrs holter. It’s important to know the ectopics burden (how much ectopic activity) only then anything can be decided.

The tracing which you have sent just show only ectopic.

Kind Regards

I will need to do on my own Dr. I will do a 48 hr and will send you as I have my own hand held one that I can record. I will also follow up with cardio when I can. My dad had a pe and cabg and uncle passed from heart attack at young age of 35 so I am now nervous. I have the pc 80b are you familiar Dr? Can I use that on myself and send you report? I have medtronic implanted loop recorder, but my Dr can not be seen at the moment due to covid. Thank you so much.

They seem to happen when at rest. When I move around I do not feel them.

Can they happen due to hormone changes?
Is it common to have them everyday? Usually only at rest.
I have mitral valve prolapse I forgot to mention
Can they be caused by a sedentary lifestyle?

Brief Answer:
Hello again

Detailed Answer:
The device which you have will only be useful if it can give the analysed summary in one page of whole 48 hrs recording as it’s not possible manually to calculate ectopic burden.

Also the family history which you have provided now is very important and it makes you a candidate for yearly health checks.

Yes, patients with mitral valve prolapse encounters ectopics more commonly as compared to other normal individuals.

You feel them at rest because your mind is free that time and it notices even small things happening, while at work your mind is occupied and they are left unnoticed.

Kind Regards
