Hi Doc, I Am Undergoing An IVF Treatment Right Now.
Question: Hi Doc,
I am undergoing an IVF treatment right now. This is my first IVF cycle. Had my ovum pickup on 26th July,2019. 5 days later, doc called me to inform me about the embryos.
I had
1. 4AA grade embryos -2
2. 3AA embryo -1
3. 2AA embryo -1
4. 3AB embryos -2
5. 4AB embryo -1
Doc advised for Frozen embryo transfer, as I am a PCOD patient, with AMH -21.9 and as a result of OVUM pickup, i had OHSS. So she advised to do it a month later.
For frozen transfer, they asked how many embryos we want to freeze, as there is cost associated with this. The higher the number we freeze , the higher the cost.
We went for freezing of 4 embryos.
I want to know, was my decision correct? Should I have gone for all the embryos.
After 31st July, Doc kept me on following medicines:
1. Aspirin
2. Estradiol
3. L-Follinine
4. Folic Acid
5. Glycomet
During this time, I had one menstrual cycle as well.
Now, my last visit was on 26th August. I had a scan. And endo was 8.1mm.
Doc said they will do embryo transfer on 2nd September.
She asked me take progestrone injections from 28th August to 2nd September and continue above medicines as well.
Now, I have following query:
1. Are my embryos good enough for pregnancy?
2. If yes, what are chances of pregnancy, inspite of very high AMH -21.9?
3. Will all my 4 embryos will be transferred in this single cycle?
4. What if I want twins, what shall I do in that case?
5. Is embryo transfer painful? Is this similar to how IUI is done?
Also , before IVF , i reduced my weight from 72 kgs to 66 kgs. However, thats not much, but still, does this help in a successful IVF cycle?
Your answers will be a great help and relief to me.

Your narration on Embryo's are confusing, hence difficult to comment on quality.
They decided to transfer embryos later to avoid complications of OHSS in view of high AMH.
BUT HIGH AMH WILL NOT HINDER YOUR FROZEN EMBRYO TRANSFER SUCCESS.
Success depends on various factors.However 20 to 50% chances are there after frozen ET if REST OF PARAMETERS ARE NORMAL LIKE ENDOMETRIAL THICKNESS .
THEY CAN TRANSFER 2 OR ALL 4.
NO ONE CAN DECIDE EARLY FOR TWINS OR TRIPLETS.IT JUST HAPPENS.
EMBRYO TRANSFER IS DONE UNDER ULTRASOUND GUIDANCE AND IT IS PAINLESS LIKE IUI.

The IVF doc told me on 6th Day post egg retrieval, that i have following grade embryos and the number of embryos in each grade.
They are as follows:
1. 4AA grade embryos -2 - Means, i had 2 4AA grade embryos.
2. 3AA embryo -1
3. 2AA embryo -1
4. 3AB embryos -2
5. 4AB embryo -1
I believe grading determines the quality of embryos right?.
Please let me know your thoughts on the quality of embryos I have.
Also, my endometrium seems to be fine , plus the uterus is also normal.
The only thing I am scared about is my PCOS. I have typical PCOS. And thats the reason I have high AMH. Coz when I started with this IVF, the doc told me, there are chances of bad quality egg and in case that succeeds, there are chances of abortion.
I just want to know, will PCOS also affect my FET success?
