Hi - Over The Past 10 Days I Have Been

Question: Hi - over the past 10 days I have been experiencing severe and unusual headaches that start immediately after sex and last at least 24 hours. Initially, the headache is very painful and makes me uncomfortable to the extent it makes feel that something is popping up out of the two front sides of my head. Then it slows down a bit but intensifies when I do exercises like taking shower or other body exercises. It goes away after about 24 hours.
The same headache comes back when I start having sex. With this new situation, I am unable to do regular sex per day as I was used to do at least 3 different rounds.

I have been taking panaldol and paracetamol 500mg to relief the pain. But none has been helpful and they are useless.

Any advice please noting that I have never had such situation in the past

Brief Answer:
You are suffering from primary headache related to sexual activity.

Detailed Answer:
Thank you for posting your query.

I am Dr Sudhir Kumar, Neurologist, and I would try my best to help you.

I have noted your clinical details. Based on this, the most likely diagnosis is primary headache related to sexual activity. Primary means there is no secondary cause. However, a brain scan may be done to exclude secondary causes such as any malformation of blood vessels.

It can be treated with medicines such as indomethacin or rizatriptan tablets (which can be taken after headache starts) for relief. Since headaches are frequent, you can take propranolol tablets to prevent headaches in future.

I hope my answer helps. Please get back if you have any follow up queries or if you require any additional information.

Wishing you good health,

Dear Dr XXXXXXX

Many thanks for your feedback.
I would like to request few details with regards to the proposed scan and treatment. As for the brain scan, do you mean MRI on the head or a particular brain scan?

Regarding the treatment, could you please share proposed doses of indomethacin or rizatriptan tablets? And could you tell me of an alternative medicine if none of the above is not available?

And lastly could you please clarify how to coordinate taking above two medicines with the Propranolol and what dose?

Thanks in advance

Brief Answer:
MRI with MR Angiogram Brain can be done.

Detailed Answer:
Thank you for getting back.

The scan needed is MRI with MR Angiogram Brain.

In my patients, I use Indomethacin at a dose of 25 mg and Rizatriptan at a dose of 10 mg, to be taken immediately after headache begins. One needs to take any one of these medicines, and not both.

Propranolol is given at a starting dose of 40 mg taken once daily after breakfast for one month.

Best wishes,

Dr Sudhir Kumar MD DM
