Question: Hello, my daughter has been had the following system for months - since she was travelling in Rome: numbness on outside of thigh. It has progressed to the other side, shoulder, neck and cheek. She says it's a tingling feeling, and sometimes actually hurts. He has seen a PA at her college and after given medication for a couple of weeks with no result, was referred to a neurologist surgeon. Not sure why a surgeon, but after a few minutes, he suggested that she get a roller backpack - not the issue. She does have Factor II thrombosis (blood clots). Also, she is at least 2 weeks over on her period - no sexual contact; and also insomnia. Do you have any suggestions? Thank you,

Good afternoon.

Your daughter suffers from numbness on the outer aspect of a thigh and this has become apparent to her on the opposite side of the body (opposite thigh as well??) in the shoulder, neck, and cheek. There are tingly feelings (paresthesias) and at times it's painful (dysesthetic/allodynic). She tried changing the backpack of the neck and back but this hasn't really helped reduce the symptoms. She is overdue with her period but is not sexually active and suffers from insomnia.

If she were my patient I would do a full neurological examination on her to include motor and sensory to map out the areas of deficient sensation (if any) and to correlate this to any limb or facial weakness (if any). I would want to check her gait and other motor functions as well as eye movements and thinking processes to make sure everything is on track (probably is since you would've said something else about her if cognition were off, right?).

I would then, send her for 2 things that need to be looked at in addition to just doing a solid neurological examination. One would be blood work to look at:

1. Thyroid hormones (FT4, TSH), Urinalysis
2. serum Vit. B12, folate, Vitamin D, ANA panel (both quantitative/qualitative), ESR, C-reactive protein, CBC with differential, HEPATIC AND RENAL ENZYMES
3. I would also LIKELY send her for an MRI of the brain without gadolinium contrast agent if we can avoid it on the first pass just to save their kidneys from potential issues. If the scan shows something funky then, we would go back and pick up the contrast agent.

Based on the results of her examination, lab tests, and imaging studies I would then, make further clinical decisions to determine whether more testing would be necessary or not. Not sure that the insomnia is contributing to anything that's going on but we could certainly also look a bit at her SLEEP HYGIENE (perhaps send her to a sleep specialist) and consider doing a SLEEP STUDY to see if she suffers from SLEEP APNEA, PLM's (periodic limb movements) that are excessive, or even RLS (Restless Legs Syndrome).

Well, I guarantee that if things don't get better soon....or if they do and then, later come back....SHE'LL make the time.....They always do! LOL.....

Now, I don't really know about the trip to Rome....was that one of those STUDY ABROAD semesters or some sort of Club Project doing an outreach program of some sort in the summer? But I'm sure she must've gotten properly vaccinated? If she spent any time in the outdoors then, if the tests I mentioned come up negative for anything the next thing I would do is check the World Health Organization (WHO) website for any health alerts they may have had out on Italy within say 1-2 years prior to your daughter going in case there is a bug, virus, or vermin that they were warning people about.....then, I would have the doctor here check into whether or not she could possibly be exhibiting anything having to do with that trip. For instance, if she went swimming in any freshwater lakes, ponds, etc.....or in the Mediterranean....she didn't bump noses with a sea critter or something did she? Does she have trouble with headaches by any chance and if so, can she relate anything having to do with any recent bad headaches with these funky symptoms she's reporting?

The other question which is related but should run in PARALLEL to all of this discussion is whether or not your daughter is doing well in school? Does she like it? Lots of friends, busy with lots of stuff? Is she into any sports or Clubs/Sororities, etc.? Is she nervous or anxious for any reason? Any guy problems that she's been dealing with? Is she worried about grades, her major, or maybe no major yet?...HA! Believe, I have 4 collegiates who all live at home. 1 just graduated from the MBA program and she's pre-med, the other graduated and was pre-med but then, decided (after 7 years of telling us he was DEFINITIVELY, POSITIVELY, UNEQUIVOCALLY, pre-med...wanted to be Neurosurgeon) that he really wants to go to BROADCASTING SCHOOL and learn how to be a Sports Cast announcer.....the other just graduated and said she wants to get her Master's....but won't just go for it...because she wants her job to pay for it! LOL.....Are you kidding I told her? I'll take care of things....just stay in school....and then, our youngest is a Junior.....and she's the MOST OBEDIENT and least difficult to control of all of them! ROFLMBO!!!! So, I know what having all of that in the house is like....and how nervous, and anxious, and everything these kids can get.....Anyways, in your daughter's case, if nothing pans out in all the tests then, the distribution of her symptoms of the numbness and so forth as well as how they're tracking could point to an anxious condition of some sort.

But by no means would I list that stress, anxiety, or any of that spectrum of problem anywhere at the top of the list of possibilities until everything's been flushed out according to what I've suggested be done first.
