Question: Hello, I have a quick question. I am a Type 1 diabetic who takes Humalog and Lantus. Last night, I can honestly say I do not remember if I took my lantus (night time insulin) prior to me falling asleep. This AM, my sugar levels were elevated which led me to believe I did not. I did take 5 units (half of my usual dosage) at 7:30AM. My question is when I get to my usual time to take lantus which is between 8 and 9 at night, should I only take 5 units and then wait a full 24 hours to take my normal 10 units or should I take my normal 10 units at night knowing I would have 11 hours remaining with the 5 units I took this Morning or how should I treat this?

I have gone thtough your query.
I can understand your concern.

Lantus Ins. is a long acting insulin that last for 24 h.
For this reason it is necessary to apply the injection every 24 h ,at the same time every day.

If you do half dosis or even forget to do the injection of Lantus in the evening,it is recommended to correct the morning blood sugar(if you have high fasting blood sugar)with a higher dosis of short acting insulin and to do the right dosis of Lantus Insulin tomorrow evening and on.

IF for some reason I did take my 10 units last night and then took 5 more this AM would I have to worry about extreme lows during the night if I retook the 10 units at 9PM?

I guess I am worried since I took 5 units at 7:30AM and usually take 10 units at 9PM that if I take my usual 10 units at 9PM tonight would I have to worry abotu extreme lows at night because of the "extra" 5 units I took at 7:30AM?

You will not have extreme lows.

Detailed Answer:
Hi again.

It is very important to take regularly all the injections of insulin therapy at the same time each day.
Because of missed dosis of Lantus last nigh,you may have troubles of you Diabetes numbers for 2-3 days.

You took half-dosis of lantus in the morning.
This is a low dosis and with this dosis of basal insulin couverure it is probable that during this day and evening you may have "higher than normal" blood sugar results .

Considering this fact,even if you do your normal dosis of Lantus in the evening,the possibility of extreme hypoglicemia is low.

Anyway,if you were my patient ,to avoid a possible hypoglicemia during the night,i will suggest to do 8UI Lantus this evening and eat a snack before you go to bed.(It is very important to check your blood sugar before each meal and before you go to bed for today.)
Tomorrow evening you will do the normal dosis of Lantus.
