Question: Hello . I had gastritis about 6 months back. I started with 2 nexium 40 mg each . I have slowly scaled down to one 20 mg nexium in the morning and one 75 mg zantac in the evening. One or two days a week, I take 80 mg nexium in the evening before having 3 or 4 light beers. Then I have 2 tums in the end.It is working. My questions are:
1) Because of adverse Zantac news, I want to switch to pepcid in the evening. Is that ok or you suggest something else. what dosage of pepcid will equal 75 mg zantac?
2)Instead of 80 mg nexium in he evening when I am drinking, should I try only 40 mg or 60 mg nexium?
3)Is it better to have tums before drinking instead of afterward?
4)I still have to take these medicines. Should I take 20 mg nexium in morning and also 20 mg in evening instead of nexium plus 75 mg zantac? will total 40 mg nexium be better and heal me faster.
5) Are 2 to 3 pegs of single malt whiskey better for gastritis than 3 to 4 light beers?
Thank you for your good advice.

Thank you for your reply.

1) Is 20 mg pepcid equal to about 150 mg zantac?
2)I took 2 nexium 40 mg each one in morning one in evening 6 months back. Then I came down to 1 nexium 40 mg plus 150 mg zantac in evening. Then I have come down in two steps to 20 mg nexium and 75 mg zantac.
You are suggesting increasing nexium to 40 mg.Will that speed the healing? How will I scale down? Wont high dosage nexium have other side effects over time. Please discuss in detail and explain your reasoning.

Thanks.

Hi,
The answer for your questions.
1. Yes
2.That is the normally prescribed dose for faster healing. That is not a high dose which can cause side effects. You can take it for a period of 4 weeks which will not cause any serious side effects.

Thanks for your prompt response.

1)I take 40 mg nexium for 4 weeks for faster healing. Do I scale down after that?
How and over what time frame?
2)Is 20 mg nexium daily very safe to take for years?

Thanks.

Brief Answer:
Answer given below

Detailed Answer:
Hi,
1. Unless the symptoms rebound on stopping the medications there is no need to scale down and you can stop it cold turkey. Alternatively, after 4 weeks, you can switch over to Pepcid 20 mg twice a day for 4 weeks and stop it after 4 weeks. I presume, you are negative for Helicobacter pylori.
2, No, not safe.

Dr.R.K
