Question: HI doctor,

I am XXXXXXX XXXXXXX This request is for my mother - XXXXXXX K XXXXXXX aged 66.
She had ovarian cancer almost 10 years back and she is fine now.
She is diabetic. Recently she is getting gout like things on her feet and currently in hand, below small finger, which is extremely painful.
Our doctor asked to get her uric acid test, which has come normal but her RBC and others looks way less which is the concern for me.
So I want to take a moderate opinion on this case from you as to what can be the issue. As she was once a cancer patient and otherwise has been happily living so far. So I do not want to her run through for every medication which may otherwise effect her. So as a general physician, what all tests I may have to do further to ascertain the issue and manage it. Is this an indication of something very dangerous to her?

You are absolutely correct about her Hb and RBC count.
But that may not be the reason for joint pains.
since the uric acid is normal it can't be gout.
I think she should do a peripheral smear for her . Have you done an ultrasound scan of abdomen recently?
Iron suppliments can be given along with vitamin D and calcium.

Thanks for the response. No, last scan was on last year may be around same time.

Her current medication is:
Glycomet 80mg 1-0-1
Pioz 15mg 1-0-0

Fourts B

Any iron and vitamin C and its dosage, you would like to suggest to her, which I can get it for her?

As you said, I shall get her peripheral smear and Ultra sound scan done.
Any gel/cream which should help with her pain currently.

You can give her Orofer xt for iron and D rise XXXXXXX weekly once for 3 months for vit D.
It may be better to find out the cause for joint pain by consulting an Rheumatologist.
