Detailed Answer:
Hello there,
Welcome to 'Ask A Doctor' service...

I've gone through your query and understand your concerns...

Well, the pathology/contributory factor could be hard motion itself as it can cause excessive straining which in turn results in the injury of the colon and rectum, and this is seen during passing motion with that kind of gush of fresh blood...

Drink plenty of water(at least 8-10cups per day), have plenty of fresh green vegetables, simple homemade foods, salads, curd based diet, avoid any form of spicy food...avoid smoking/alcohol strictly if at all do...

Take Sitz bath twice to thrice daily (sit in a tub of lukewarm water putting some salt in it)...You can watch a tutorial video of Sitz bath online for proper techniques...

Apart from this, do not strain while passing motion...For constipation, you may take Syp. LACTULOSE 15ml once daily at bedtime and observe how you stay (the dose can be further escalated up to 30ml thrice daily based on the initial response)...

In case you've already undergone any routine colonoscopy check-up earlier (and if they're clear), then I wouldn't think of anything worse at this moment...

Although the pathology seems to be hard motion, there can be an associated Fissure/Hemorrhoids (if it's painless during passing motion, if there's no itching, no lumpy feeling then it could be internal haemorrhoids).

However, the final diagnosis can only be ascertained after a per-rectal examination, so it'll be better to get it examined clinically by a Gastroenterologist or a Surgeon for further evaluation...

Let me know if I can assist you further.

Take Care
Kind Regards
