Is A Finding Of BiRads 3 In A Breast Ultrasound Indicative Of A Need For A Biopsy Rather Than An MRI?
Question: Is a finding of BiRads 3 in a breast ultrasound (diagnostic mammo was BiRads 4), indicative of a need for biopsy next rather than an MRI? The finding in my regular and diagnostic biopsy was a 'possible subtle architectural change'. The radiologist recommended a breast MRI with and without contrast next. If that is negative, then a mammo at 6 mos. If it is positive, then a biopsy. One surgeon I met with suggested to go right to a biopsy and skip the MRI; however, I am concerned that they don't see anything in the ultrasound (other than a couple of cysts that have been there before).
my mother had breast cancer at nearly 60 years of age. I have no previous history.....

Brief Answer:
Better to go for the biopsy.
Detailed Answer:
Hello,
BI-RADS (Breast Imaging Reporting and Data System) is the system used to assess the risk of malignancy(cancer) risk on the mammogram.
BIRADS-4 is suspicious and tissue-based diagnosis is recommended while BIRADS-3 denotes probably benign and followup after six months is recommended.
Since your first mammogram showed BIRADS-4 and considering the family history it is better to go for tissue diagnosis.
A surgeon takes a small bit of lump through a needle and it’s a minor procedure and nothing to worry. Further treatment will be based on biopsy report.
Wishing you good health.
Thanks

Is it helpful to start with an MRI, then proceed to the biopsy? I am in a waiting mode for three weeks to see the specialist I would like to do any procedures.
Brief Answer:
MRI not indicated.
Detailed Answer:
Hi,
No need of an MRI.
BIRADS4 itself is an indication for biopsy. Since you have a family history, early biopsy would be more beneficial.
Do not worry about the biopsy, it’s a small procedure done in 5 minutes.
