I am Dr Ajeet SINGH.

I have gone through your query and here is my opinion.

1)
The tentative diagnosis according to me seems Chronic Granulomatous disease.
Most of times it is infective (needs antibiotic,anti protozoan and anti fungal treatments) but it might be non infective.

A granuloma is an end stage structure formed due to accumulation of immune cells (macrophages) in response to a foreign body or an infective agent.

Granulomas form when immune system circumscribes and attempts to wall off the f.body which it is unable to eliminate.

CGD is a spectrum of granulomatous conditions with diverse etiology but common histology and at times etiologic agent is not even identifiable. making an accurate diagnosis very difficult.

2)
To ascertain the exact prognosis,you need to be subjected to extensive clinical examination and multiple tests like PFT,PCR Tests,culture and serological tests ,HRCT Chest and Lung/Lymph node biopsy.

Complications may be breathlessness,Pneumonia, suppuration adenitis,secondary infections,osteomyelitis and lung failure etc.

3)
Your report shows a regression of disease and an improvement in response to treatment.
Such response to medication is indicative of an infective etiology and a much better prognosis and better chances of resolution of lesions.
