Dear Dr

My mother has following ailments
A Diabetes 2. Since 10 years
B. BP. 20 years
C. Nerve pain goes in left leg since 2 years
D. High triglycerides



Her weight is 230lb. She is not exercising due to leg pain. And neither dieting.

RECENTLY HER RFTS ARE NOT WITHIN THE NORMAL RANGE.

Please advise. Reports of the blood work are attached.

Hello and welcome,

I have read your query and here is my advice.

In my opinion, uncontrolled diabetes, uncontrolled BP and some medicines can affect kidneys.
In your mother's case, diabetes seems under control. I don't know about her BP control but you may want to keep a regular check on it. Maybe have a BP monitor at home.
Amongst the medicines listed, hydrochlorothiazide (one of the ingredients in triforge), esomeprazole (nexum) and lipiget (atorvastatin) have some association with kidney problems and need to be avoided. I think that if we withdraw lipiget and nexum, it will not be an issue. Regarding use of hydrochlorothiazide, you may check with her physician and corelate with BP findings too.

Keep a regular check of serum creatinine levels and if deteriorating, then she may need to see nephrologist or dialysis centre as required.
