Question: Dear Doctor, our baby girl was born on 6th august. her birth weight was 2.4 kg. in six days baby lost weight and her weight on 11th aug was 2.140kg. The doctors said direct breastfeeding should be given every 2 hrs and expressed breastmilk should be given 6 times in 24hrs. Then baby weight increased to 2.240 kg on 14th aug. the same we got discharged from the hospital. In home we were giving 30 ml expressed breastmilk every 2hrs in night and 30ml expressed breastmilk every 4 hrs in day time. yesterday that is 25th aug baby weight was 2.555kg. but expressing breastmilk is very difficult and is painful for mother so we were giving only direct breastfeeding for 15 to 20 mins. but the baby is crying for milk every one hour. it is very difficult for the mother to feed the baby every one hour. we are confused now. please advice us as how often we should feed the baby. also in the hospital doctor said if we give direct breastfeeding for long time baby will lose energy and there will be weight loss. we are confused now. please advice us on this. thanks.

Brief Answer:
Suggestions on breastfeeding

Detailed Answer:
Hello... thank you for asking a query here.

It is very much physiological for all the babies to lose 10% of their body weight and gradually regain this wait by bye 10 or 14th day.

My suggestions for the baby are that you feed the baby every 2 hours once and in between if the baby is crying every 1 hourly.... first try to console her by lifting the baby up and soothing her.

I am telling this because every time a newborn baby cries it is not feed and it could be for human comfort and touch and also for mother's voice smell and sight.

Even with this if she is not coming down then you can use formula feeding if mothers feed is not adequate.

Butut if the mother is having dripping of milk from one breast when she is feeding from the other breast and when on exclusive breastfeeding if the baby is gaining weight appropriately and urinating 4 or 6 hours once, that means that mother has got adequate milk production.

Another important concept to keep the baby's stomach full is the concept of foremilk and hindmilk.

Fore milk which is the initial part of breastfeeding is sweeter to make the baby latch on well and the milk which comes from the last part of the feeding is rich in fat which is called hindmilk which will make the baby stomach full and also help the baby gain weight.

So my suggestion is that when you are feeding the baby it is best to feed two times consecutively from the same breast (so that both foremilk and hindmilk are fedd to the baby well) and then when feeding from the other breast again 2 times consecutively.

Try this for two weeks and you will definitely find good results.

Hope all these suggestions help!
