Question: DR

Disabled from
Resection of Spinal Cord intermlgy Hemangioblastomas
October 2013 @C2-C3 I have all the typical post surgical symptoms primarily on the left side of my body
Including pain, spasms, numbness, upper body swelling, trigger finger both, hands some balance etc

I’m hoping you can direct me where to study this “new symptom”? And or a the clinical term/name for this particular condition.
If warranted, ie.. I’m in danger of losing what mobility I still have would you advise me to pay $$$ go out of my network to see a neurologist soonest.

For the past month or so experienced what I call a lighting volt or jolt that I feel going down my (L) leg and slamming the bottom of my foot, when I yell? as when screaming for my dog to be quiet.
Recently I’ve been able to duplicate this system by slapping the back of my neck.

Thank you Dr.

Brief Answer:
The spinal cord had been encroached.

Detailed Answer:

Hi,

I have gone through your query and understand your concerns. As I understand, you had developed hemangioblastomas encroaching the spinal cord and that they were resected at the C2-C3 level. It seems that there was some nerve involvement during this process of encroachment and the surgery could not resolve part of the damage already caused. Our brain has an inhibitory action on our muscles, under normal circumstances. These connections come down the spinal cord in form of various tracts. Some of these tracts have been damaged at a certain level.

The loss of the connection from the brain therefore leads to removal of this inhibition, thus leading to unopposed contractions or spasms, and the resultant fatigue of the muscles leads to accumulation of lactic acid leading to pain. The spinal tracts also are responsible for certain fine senses like balance. Needless to say, balance is affected. Nerve compression leads to parasthesia or numbness and that is what you are experiencing. The autonomic nervous system controls the blood supply and changes in it are related to the swelling. All pieces point to the same pathology.

See your neurologist and I hope that he would be of some help.

Feel free to write back.

Thank you Dr.

My question was about the (sciatic type) Nerve
Activity that is triggered when I yell out.
My puppy XXXXXXX barks a lot, so
several times a day I scream quiet.
I’ve never heard of voice related nerve pain

Is there a name for it?
Where can I read more about it?
My new Medicare Advantage Plan can’t even provide
Me with a PCP much less a referral to
A neurologist.
If and when I do see a Dr I would have to convienve
him to approve referral thus as much knowledge
As I can learn study beforehand would be
Beneficial.

Just an FYI

No treatment attempts since 2017
Pain Management Department team quit with out
Notice Possibly falling back on (Do No Harm)
2016 they tried Botox for the back spasms seemed to work
I was walking better further but it wore out
I did not make it out of the hospital
After the 2nd treatment 500ml and had seizure
Type symtoms for months following.
I won a state grievance Medically Necessary VS Not
For a Baclofen Pump that has been recommended
By top specialis. I had an appt for trial that disappeared
From my on line portal 7 months prior.

The Departmet Head I finally met with said “ I wouldn’t let any of y Drs within 100 mi. Of you” so if you insist on this procedure being done were
refer you elsewhere.
I decided that maybe it wasn’t worth it and didn’t do it

Brief Answer:
You may give acupuncture a try

Detailed Answer:
Shouting can be associated with tensing of muscles, thus leading to muscle spasms and pain.
I do not find a correlation between boro and seizures.
See a different doctor (local) and discuss if he thinks that you would benefit from a baclofen pump.
