Question: I am a type 1 diabetic, sometimes I have an attack that suddenly happens with severe abdominal cramps, nausea, diarrhea, sweats and feel as though I am passing out. Typically it ends in 30-45 minutes. Several days prior to the attack I notice a fruity scent in my urine; sometimes numbness in right leg and back area.

I have gone carefully through your concern.
Having into consideration your history the most probable cause is of neurologic origin. Autonomic neuropathy is the most probable cause, but usually it is associated with other chronic autonomic neurolopathies caysed by Diabetes.
Of course a detailed clinical examination is very helpful in these cases and keeping normal glucose levels helps in preventing further deterioration of the chronic complications.
Thyroid, adrenal function should be checked also.
Disscus with your treating physician about autonomic diabetic neuropathy.
