Question: 61 y/o male with knife like pain 2cm to the left of vertebral column at hip level - only evident upon standing or walking for more than 5 min- immediately alleviated by sitting. only treatment has been low dose prednisone 5 mg /day alleviates 50%25 of symptoms- have not tried PT or other modalities of therapy- my concern is with impingement on L5 nerve in 2 areas will i require surgery- thank tou XXXXXXX

I have seen the attached MRI scan report. There is a nerve impingement and mild stenosis of the foramina. There are degenerative changes in the spine that happen due to age related wear and tear. Any surgery is not immediately needed. This can improve with the help of medicines and further progression can be considerably retarded with the help of regular physiotherapy. Even if the symptoms reduce, do go for a regular physiotherapy so as to delay the surgery as much as you can.
