Question: 3 months ago I started to get gastritis symptoms and esophagitis in the lower 3rd of my esophagus. At the same time, I was put on a muscle relaxer because my back was really tight and hurting me. The muscle relaxer didn't help, but I noticed when it wore off my heart rate went up and and my blood pressure went up. It all went back down again at after awhile. I noticed that whenever I had food in my stomach my heart rate would be erratic going up over 100 while sitting on my couch, but going back down. Fitbit had my resting heart rate at night in the 70s-80s, though. I always tried not to eat late before bed so my rate would go back down. This persisted from the beginning of XXXXXXX to just 2 weeks ago. At that point I noticed my resting heart rate on Fitbit was going up to 91 at night. Around that time, I used an ice pack on my chest and pretty much left it on until it went warm. I was trying to cool the top of my stomach off and my esophagus. I tossed and turned and noticed the next morning that my resting heart rate was 115 bpm in bed. So, I went to the hospital where they just thought I was being anxious. I went to the doctor, and they gave me 10mg of Propranolol to take 2 times per day. Even while on the Propranolol I noticed that my blood pressure was also getting high. The hospital wouldn't do anything about that, either, so I scheduled an appointment to see a cardiologist. I never had any heart problems whatsoever before this gastritis event. I have a few theories on why my heart is out of control. 1. I wonder if the ice pack damaged my sinus node or could have sclerosed some arteries. 2. I was wondering if a tumor could be pressing on an artery or something directly effecting my heart. 3. I was taking some Celexa (supposedly to aid with irritable bowel that I don't even think I have) at the time and I had already been getting what feels like adrenaline or hormone surges in my gut/ bladder/ or pelvis. This seems to happen randomly and it makes me feel weird all day. 4. I stressed and worried so much for 2 months that my heart is messed up from that. What is most likely/ least likely? I am concerned that I have some kind of cancer being that my stomach and bowels have been uncomfortable all summer and I have lost about 20 lbs since the end of May without intentional exercise. I have normal sinus rhythm so far, just fast. I am waiting on a 2nd opinion of CT where they found a few "benign liver cysts" I am not sure are benign. Concerned they missed something in the pancreas or there could be hormone producing tumors. I was told my body produced excess DHEAS years ago when I had irregular periods, but I figured I was fine when they became regular. Now, since 3 months they are irregular again. What tests should I ask expect/ ask of the cardiologist that I seen in a week?

Hello!

Welcome to Ask a Doctor service!

I passed carefully through your question and would like to reassure you that it is less likely that the ice pack has damaged your sinus node or coronary arteries.

I would exclude any tumour too, considering all your performed tests.
In my opinion the main issues to consider are anxiety and Celexa, which is commonly related to tachycardia and ectopic heart beats, as its adverse effects.

For this reason, I would recommend switching to another antidepressant, which does not affect the heart so much ( ex. sertraline of paroxetine).

I would also recommend checking your thyroid hormone levels for possible thyroid gland dysfunction, your blood electrolytes for possible imbalances and checking your blood or stool for the presence of Helicobacter Pylori, which is an infection that leads to acid reflux and weight loss.

Regarding your heart, I would recommend performing a cardiac ultrasound in order to examine your heart function and structure and an ambulatory 24-48hour ECG monitoring in order to examine your heart rhythm trends for a prolonged period of time.

Regarding the irregular menstrual cycle, it could be related to anxiety and the weight loss. Nevertheless I would recommend checking sex hormone levels and prolactin plasma levels.

You should discuss with your Doctor on the above tests.

Hope you will find this information helpful!

Wishing all the best,
