Urinary Pain While Urinating?

Hi my son is having redness around the opening of his penis and it hurts while urinating. He is not sexually active. I know this for a fact he is always with me. He is handicapped also. The doctor said it is urethritis. But I looked that up and it is all about sexually transmitted diseases. What could it be for someone not at all sexually active. Masturbating or otherwise? They also checked for a bladder infection and the results were negative.

If its a urethritis. Then he'll probably will have other symptoms also of urethritis. Did he injure himself. Injury will cause redness and also without the symptoms of infection
