Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

You may be suffering from decompensated cirrhosis of liver with generalized edema of the body. Yes it may cause resistance ascites with groin edema. He may need high dose of Lasix and Aldactone in 10:4 ratio.
Maximum dose of it is Frusemide with Aldactone.
Still symptoms not improved he may need peritoneal tapping with transjugular intrahepatic porto systemic shunts.
Still no improvement, need liver transplantation.

Hope I have answered your query.
Let me know if I can assist you further.
