"35-year-old man who smokes two packs of cigarettes daily" appears to be at high risk for respiratory complications as lungs are already compromised due to smoking.
Option "A" & "B" may also be at high risk in long run.
Persistent diabetes and Hypertension may cause renal shutdown by chronic kidney disease and may manifest in form of respiratory distress.
Hypertension may also cause effect on heart and lungs and may cause respiratory distress.
Chronic allergic rhinitis by involving lower respiratory tract may cause respiratory problems.
