What Is The Treatment For Phimosis?

Hi. This question is for a male urologist specializing in issues pertaining to phimosis. Many thanks for understanding and any help in advance

I am a 25 year old male, and about 3 months ago I self-diagnosed myself with phimosis. Please allow me to explain my case in some detail

Due to a lack of public awareness about intricacies pertaining to this topic, up until recently I seriously did not know that the foreskin should be fully retracted during sex or masturbation for non-circuimsized men. I would receive adequate stimulation with the foreskin remaining mostly over the glans, so I just assumed it was normal

I have not yet had intercourse before, but I do masturbate relatively frequently. I also do not typically pull back the prepuce whilst urinating.

I suppose that a combination of the above factors are what led to both my frenal ring and my frenulum not growing/stretching adequately enough over the past few years. I do remember the prepuce properly detaching from the glans in my late teens -- I discovered it happening while in the shower one day, where I discovered that I could now retract further than normal and see parts of the glans that I had never seen before. So naturally I helped it along over the coming days, and it wasn't long before I could retract completely when flaccid. I still did not take this to mean that I should retract whilst erect, however.

A few years have gone by since then, and I recently realized that it would no longer retract as easily, even while flaccid! Perhaps shrinking a bit? So, out of concern I researched the issue online, and discovered that I basically had phimosis and that the foreskin was supposed to be retracting in cases where I had neglected to allow it. Whilst I was somewhat fortunate that my case was not too extreme, I still knew that I was supposed to be able to retract easily even whilst erect, but of course that was now impossible!

So I learned about the stretching exercises, and have been doing so twice daily now for the past few months (without any topical steroid creams however). There definitely seems to be an improvement, as I can now retract about 80% of the way whilst fully erect (not completely behind yet though as this is too tight on the ring), which would have been impossible before. All the pressure does seem to be taking its toll, as pulling back is still sometimes very painful on the glans, as if it is somehow a bit swollen inside

Today however, whilst masturbating, I may have expected too much and allowed more pressure during masterbation. I then felt a sharp prick/snap sensation and saw a gush of blood. Fortunately there was minimal pain otherwise, and penis went flaccid almost instantly and the bleeding stopped rapidly as well, so I needn't panic much. I realized that I had snapped my frenulum however and the two sides are now detached!

Since my frenulum was a bit short and inhibiting proper retraction before, I am wondering it I should allow it to reconnect, or if I should keep them apart during healing. I understand that this may be a personal choice, however I would appreciate any advice on this. Whilst I normally keep my foreskin over the glans in rest position, I have chosen for now to keep it retracted during the healing process, to encourage it to heal back longer than it was before. When retracted, the distance of the snap is maybe 5mm, and there is a bit of blood in that area between the two sides now. From the research I've done, it seems even if it does not heal back together, that should not be a problem and in some cases is desirable?

My main question then, is as follows: Should I rather let it heal with the foreskin intact (with the risk of it healing too short again), or retracted (with the risk of potentially losing the frenulum altogether)?

I apologize for imposing with these personal matters, but I couldn't find a specific answer to that question online and would appreciate if a professional could advise me as it will also potentially help others with the same issue

Thank you
