The treatment of any type of subdural hematoma depends primarily upon the size of the clot, whether or not it's causing significant pressure against the brain and what the clinical status of the patient is at the time that it is found. If it is small, without mass effect on the brain, and the patient is lucid without neurological symptoms then, it is perfectly reasonable to take a watch and wait attitude. If the hematoma is large and causing the brain to shift inside the skull causing either severe symptoms neurologically or causing the patient to be very sleepy, tired, confused, unbalanced to the point of being a falls risk then, the clot will need to be evacuated.
