What Is The Cause And Treatment For GERD?

My Wife having below problem 1) With the burp (Dakaar aana) food comes back in mouth with water. 2) Feeling uncomfortable on neck like something pinning in neck. 3) 3 year back Thyonom 12.5 mcp, dose was started and after consuming the 6 month, she get normal and then she stopped to take medicine. 4) but again in the month of march 2014 this year while she tested TSH found the following results as below. Test on date 04/03/2014 TT3 = 105.79 ng/dL TT4=5.2 µg/dL TSH= 6.28 µlU/mL Method: - CHEMLUMINESCENCE Again check on 14/03/2014 Serum Prolactin = 6.27ng/ml (method done by CMIA) on ARCHITECT. TSH(VIDAS) = Done by ELFA on Vidas (Method : Done by ELFA) on Vidas PC. *After above check on 14/03/2014, Dr. Sugessted to take Thyronom 75 mcp. **After consuming the above medicine 80 days Thyoronom 75mcp, she was feeling very very uncomfortable and finally admitted and while tests the following results are found, please see the below results. Date 05/06/2014 IMMUNOASSAY PTH-(Intact-Molecule) = 16.5 pg/mL (Serum. CIMA) SERUM CALCIUM =7.49 mg/Dl (Method : OCPC/ASZ III) SERUM PHASPHORUS =4.05 mg/Dl (Method : Phaspho Molybdate) TSH = 0.05 ulu/ml •     Then stopped the thyronom immediate and started CAL SHEL -20 mg and Soduam medicine VEL D2- 20 Mg And again checked on 09/06/2014 and following results found T3 =3.98 nmol/l T4 = 14.4 ug% TSH= 0.13 uIU/ml Still she is not taking Thyronom but Dr. suggested to start after check again now.

Hi ! Good evening. I am Dr Shareef answering your query.

If If were your doctor, I would recheck your TSH after another two months before staring the thyronorm. If the TSH increase, then I would start the Thyronorm under controlled supervision.

On the GERD front, I would advise the following for you :

1) Elevate the head end of the bed with two blocks on each side, so that the regurgitation does not occur while lying down, and the contents of esophagus and stomach flow down with gravity.
2) Some modification in your diet like more of roughage, enough liquids, avoidance of oily, fried, and fast food
3) Regular routine exercises like morning walk depending on your medical fitness.
4) Prescription for a prokinetic agent and a proton pump inhibitor for symptomatic relief and to have a control on the acid production and facilitate movement of gastric contents downwards.
5) A routine blood sugar test both fasting and post prandial.
