General & Family Physician 's  Response
Hello dear
Understand your concern
Presence of E coli in semen/sperm suggests infection in male genital system.
You have to take the doxycycline and nitrofurantoin for 3-4 week to clear the infection.
Your sperm count is very low and with this natural pregnancy is not possible with this sperm count.
You have to consult the urologist for proper evaluation.
Following are the causes of low sperm count:
Low testosterone, blockage in the sperm tubes, smoking and alcoholism, ageing, exposure to heat and toxin, mumps in childhood and genetic problem.
Treatment is depend on the cause after proper investigations like Color doppler USG of scrotum, hormone analysis, CBC, ESR, urine routine and microscopic examination, PSA, Oxidative stress marker like 8 oxogauinine in sperm.
Lat option is IUI /IVF for your problem if not responds to any treatment
Hope this may help you

Contact HCM for any query
Best regards
