MRI - BRAIN (74yr old female)
Protocol : Multi-planar, multi-echo MRI of brain was performed with routine sequences.
Observation:
Marked discrete & confluent T2W and FLAIR hyperintensities seen in bilateral deep hemispheric white matter on T2W and FLAIR sequences with no evidence of diffusion restriction – Suggestive of multifocal ischemic changes.
Mild age related cortical atrophy with prominence of ventricular system and subarachnoid spaces.
Rest of the cerebral parenchyma shows normal signal intensities.
Basal ganglia and thalami appear normal. Brain stem and cerebellum appear normal.
The VII-VIII nerve complex seen normally on either side with no definite mass lesions seen in the cerebello-pontine angle region.
Sellar, suprasellar and parasellar regions appear normal.
Flow voids of the intracranial portion of the ICA, basilar artery and the superior sagittal sinus appear maintained with no evidence of thrombus noted.
Visualized portions of the paranasal sinuses and orbits appear normal.
Atlanto-axial articulation appears normal with no features to suggest basilar invagination / Chiari malformation identified.
IMPRESSION:
• Marked discrete & confluent T2W and FLAIR hyperintensities in bilateral deep hemispheric white matter – Suggestive of multifocal ischemic changes.
• No evidence of trauma related brain injury noted.
