Hello
Thanks for writing to HCM

USG findings are suggestive of renal parenchymal disease. It is supported by increased serum urea and creatinine level.Renal parenchymal disease is due to chronic diabetes mellitus and hypertension.
It is a manageable condition but he should follow diet and fluid restrictions.
He also need few other investigations like routine hemogram,Serum calcium, chloride, bicarbonate, phosphate level,GFR,Creatinine clearance.
Further treatment depend upon findings.
He should consult nephrologist.

Hope I have answered your question.
Take Care
